Protected Disclosures Report Form
The Protected Disclosures Act 2014 (as amended)
provides certain protections for workers who report information about potential wrongdoing in their workplace.
This form can be used by workers in the public, private or not-for-profit sector who wish to report potential wrongdoing, encountered in the course of their work, on matters relating to the assessment, collection and management of taxes and duties or the implementation of customs controls.
When making a disclosure please include as much information as possible.
By providing your contact details, you agree that Revenue may contact you to acknowledge the disclosure, and to clarify any information contained therein.
Revenue accepts anonymous disclosures, however, it is not possible to issue an acknowledgement for such reports.
